Case Management

We provide intensive case management services to support individuals in accessing and navigating essential resources. Our person-centered approach empowers individuals to make informed decisions and achieve greater independence through:

Community Choice Guide (CCG)

Our CCG program assists individuals transitioning to different levels of care, including independent living. This program provide intensive one on one relocation support for individuals moving from Long-term care settings to lesser levels of care including Assisted Living, Adult Family or independent living.

Foundational Community Supports (FCS)

Connecting individuals to housing and employment services that promote stability, self-sufficiency, and overall well-being

Client Training Services

We also offer client-centered training services designed to enhance independence and improve daily functioning. Training may include:

  • Adjustment to serious impairment
  • Maintenance or restoration of physical functioning
  • Self-management of chronic disease and personal care needs
  • Skill-building to address mild depression
  • Development of effective communication and collaboration with care providers

Health Home

The Health Homes Program is a person-centered, community-based care coordination service for eligible Washington Medicaid members with chronic health conditions, including diabetes, asthma, heart disease, and behavioral health disorders.

Our program promotes continuity of care by coordinating services across physical health, behavioral health, and community support systems.

Care Coordination

Our Care Coordinators help members navigate the healthcare system, access community resources, and improve their overall health and well-being. Through collaborative care planning, care coordination, health education, and referrals to community services, we support individuals in achieving their health goals, increasing self-management skills, and maintaining greater independence.

Outpatient Behavioral Health

Our outpatient behavior health  provides a structured, higher level of behavioral health care while allowing individuals to continue maintaining their daily responsibilities and routines. The program is designed to support recovery, stability, and improved functioning through a combination of services, including:

Intensive Residential Treatment (IRT)

Our team provides structured, wraparound, 24-hour care in a supportive environment for individuals with complex behavioral health needs through :

  • Bio-Psychosocial Assessment and Treatment plan
  • Crisis intervention and stabilization support
  • Individual and group counseling Medication management and coordination

Behavioral Support Services

This service provides one-on-one support and brief training sessions for long-term care clients and their caregivers, focusing on strategies to effectively manage challenging behaviors within the client's placement environment while promoting positive behaviors, developing coping skills, and improving daily functioning.
