Community Choice Guide helps transition planning
Our CCG program assist individuals transitioning to different levels of care, including independent living. This program provide intensive one on one relocation support for individuals moving from Long-term care settings to lesser levels of care including Assisted Living, Adult Family or independent living.

Health Homes improves health equity at the community level
Our Health Homes program is a person-centered, community-based service that provides care coordination to eligible Washington Medicaid individuals with serious chronic conditions, like diabetes, Asthma, heart diseases or behavior health illnesses. We support the continuity of care for physical, emotional, community, and developmental needs that an individual can experience.
